There have been some historic breaks and/or losses including to the section lower right of eye and several blood vessels but not having another comparable complete model it is impossible to be exhaustive regarding where and how many these might be; there is no guide to correspond with the numbered sections; likely early-mid 20th century, the partial label to the reverse reads 'Teaching Aid'; additional images available illustrating general condition and issues
